RELEASE OF STUDENT INFORMATION

The Federal Family Educational Rights and Privacy Act of 1974 (FERPA) grants students certain rights with respect to their education records, one of which is the right to provide written consent before the University discloses personally identifiable information other than "directory information" from the student's education record. If you choose to share your academic record you must first submit written authorization through the form below.

FERPA Directions

  1. You will need to log into your MyUSAO account if you have not done so already.
  2. Once logged in, navigate to Academics at the top.
  3. Click on Academic Resources in the side navigation.
  4. Click on “Set FERPA Permissions.”
  5. Click on “Define New Permissions."

Request to Withhold Directory Information

  1. Log into your MyUSAO account if you have not already done so.
  2. Navigate to Academics at the top.
  3. Click on Academic Resources in the side navigation.
  4. Click on “Request to Withhold Directory Information.”
  5. Click on “Define New Permissions.”
